Nurse Health Navigator
After you’ve seen your provider, do you wish you had someone to answer the questions you forgot to ask? Have you ever received a diagnosis that left you with more questions than answers?
The Nurse Health Navigator at Allegan General Hospital can help you navigate these questions and chart a course toward optimal health. The Nurse Navigator is a specially trained nurse that is available to guide you and your family through the sometimes confusing process of navigating the health care system. This compassionate professional is a single point of contact who will be there with you every step of the way, regardless of your medical condition, ability to pay, or background.
Whether it is getting patients and caregivers the information they need to make treatment decisions and better understand their disease; helping patients deal with the day-to-day challenges, such as transportation and insurance issues; or connecting them with community resources such as support groups, AGH’s Nurse Health Navigator can provide help.
AGH’s Nurse Health Navigator can be reached at (269) 686-4332. Office hours vary, Monday through Friday, so please call for an appointment. Evening and weekend hours can be arranged as well. There is no charge to the patient/family for this service, and no referral is needed.
