  • Just because a physician orders a test does not mean your insurance will cover the service. Please be knowledgeable of the specifics of your insurance coverage. In the current healthcare environment, most insurances require that elective services be pre-authorized. Obtaining pre-authorization for an elective service is your responsibility. Failure to do this may result in partial or complete rejection by the insurance.
  • Each visit to the hospital has a separate patient account number assigned to it. Please be aware of this as you match your insurance payments with the hospital accounts. Also, an insurance card is expected at each registration.
  • The minimum charge for services rendered in the hospital emergency room is $85.00. Uninsured patients are expected to pay this minimum charge at the time of service.
  • Allegan Medical Clinic is separate from Allegan General Hospital. The clinic bills for its own services. Please make sure that inquiries regarding billing/payment are referred to the correct business office.
  • Bills for emergency services at Allegan General Hospital do not include the professional fee for the ER physician.
  • Bills for operating room services include the professional charges for the anesthesiologist. (CRNA)
  • You may receive separate professional bills from a radiologist, pathologist, or other “consulting” physicians.
  • This notice is for non-parent/guardian caregivers. If you are taking care of minors at the request of the parent or legal guardian, you should have a written consent to treat form signed by the parent or guardian. (Click Here for a Printable Form)

    The law states that an adult presenting a minor for treatment is responsible for the charges incurred in the treatment. The form authorizes the caregiver to present the minor for treatment and provides insurance information and other medical information that may be required in an emergency. The form may be obtained through our Emergency Room or Central Registration departments.
