My Hospital Records
If you have been a patient at Allegan General Hospital, these records can be obtained through the Medical Records department located on the lower level of the hospital. Our Medical Records department has on-site access to most records within the past 10 years. Older records are available, but will take longer to retrieve.
Medical records include laboratory reports, radiology reports, emergency room visits, outpatient surgeries, and inpatient/overnight stays at Allegan General.
A Consent to Disclose Confidential Information form must be completed by the patient or in case of minors their custodial parent or legal guardian before records can be released. You will be asked to provide a driver’s license or a photo ID. A copy fee may be charged for your medical records. We will require 24 to 48 hours to obtain the records.
You will need to provide the staff with the following information:
Name of patient, date of birth, dates of services you are requesting records on, who is to receive the records, and reason for obtaining records (i.e. personal, physician visit, etc.)
We can be reached Monday through Friday 8:00 a.m. to 4:30 p.m. at 269-686-4191. If you call after hours, please leave a detailed voice mail message.
If you wish to obtain a copy of a birth certificate for yourself or child, and you or your child were born at Allegan General Hospital, please call the County Clerk’s office for Allegan County at 269-673-0450.
The clerk will provide you with a certified copy of the birth certificate. A fee for this service may be charged.
